腰-硬联合阻滞麻醉对产妇皮质醇、IL-10、IL-18水平的影响

目的：探讨了腰．硬联合麻醉对产妇皮质醇、IL-10、IL-18水平的影响。方法：60例足月孕妇随机分为腰-硬联合阻滞（CSEA）、分娩镇痛组（A组）和未行镇痛的对照组（B组），每组30例。A组行CSEA（0．1％罗哌卡因＋2μg／ml芬太尼）分娩镇痛，两组产妇均采用放免法测定皮质醇，酶联法测定IL-10、IL-18水平。结果：A组皮质醇、IL．10、IL—18水平明显低于B组（P〈0．05）。结论：CSEA能有效地减轻疼痛应激反应，有利于患者的细胞因子的平衡。

Effect of Combined Spinal-epidural Analgesia on Serum Cortisol,IL-10, IL-18 Levels in Patients with Pregnant Women

Objective To explore the effect of combined spinal-epidural analgesia of serum cortisol, IL-10, IL-18 levels in patients with pregnant women. Wethods 60 cases in spontaneous labor were randomly divided into 2 groups with 30 eases each. The patients in group A were given CSEA with fentanyl 20μg in labor analgesia and ropivacaine（0.1% ） combined with fentanyl（2μg/ml） inepidural analgesia. The patients in group B were not given labor analgesia as the control. Serum cortisol（ with RIA）, serum IL-10, IL- 18 （with ELISA） levels were detected in pregnant women. Results The levels of cortisol, IL-10, IL-18 levels significantly lower than those in B groups （P 〈 0.05）. Conclusion CSEA could shorten the active delivery reaction, and may benefit the balance of cytokines in patients.
