寰枢椎损伤影像学综合诊断

摘    要:目的分析比较平片、直线断层、CT和MRI检查在寰枢椎损伤诊断中的价值、局限性及其在临床中的应用。方法回顾性分析33例寰枢椎损伤患者的影像资料,并进行对照分析,比较各项影像检查结果在诊断中的价值、局限性及在临床诊断中的作用。结果本组病例中,寰枢椎骨折29例,单纯寰枢关节脱位4例。33例均经平片检查,可确诊24例;21例经直线断层检查,19例可确诊,Halo-Vest外固定架术后复查28次,图像效果满意,并在断层下调整角度,达到满意为止;25例经CT检查,可确诊24例;16例经MRI检查,13例可确诊。结论不同的影像学检查方法各有其优缺点,正确选择有助于寰枢椎损伤的早期诊断和治疗。

Abstract:Objective To analyze the value, limitations and its clinical application of plain film, tomography, CT and MRI in the imageological diagnosis of atlanto-axial injuries. Methods The findings of plain film, tomography, CT and MRI of 33 cases of atlanto-axial injuries were reviewed retrospectively. The value,limitations and its clinical application of the different images were compared with each other. Results Of the 33 cases ,29 cases had atlantoaxial injuries and 4 cases had isolated atlantoaxial dislocations. All patients took plain films and their final diagnoses of 24 patients were made. 21 patients in this series took tomography and their final diagnoses of 19 patients were made. 25 patients took CT scans and their final diagnoses of 24 patients were made. 16 patients took MRI scans and their final diagnoses of 13 patients were made. Conclusion Different imageological examinations have its advantages and disadvantages and correct choices of them are helpful for early diagnosis and treatment of the atlanto-axial injures.
