摘    要:目的探讨大鼠不同体积肝移植早期肺组织内诱导性一氧化氮合成酶(iNOS)的表达及意义。方法建立全肝、50%及30%体积肝移植模型,分别于术后0·5、2、6、24h处死大鼠,同时以假手术组作为对照,RT-PCR和Westernblot法检测肺组织内iNOS的表达,同时进行肺组织病理学检查。结果(1)iNOS表达:假手术组mRNA没有或微弱表达,各肝移植组表达增高,全肝移植组和50%肝移植组的表达水平在移植后6h时点均明显高于30%肝移植组(P<0·05);iNOS蛋白表达与mRNA表达基本一致。(2)病理学检查:假手术组肺组织结构基本正常,肝移植组出现肺间质明显出血充血,肺泡间隔明显增厚,中性粒细胞浸润明显,以移植后2h时点最为明显。结论肝移植早期肺组织内存在iNOS表达升高,可能与肝移植后肺损伤的发生有关,其表达高低可能与移植肝脏体积大小有关。

Abstract:Objective To investigate the expression of inducible nitric oxide synthase ( iNOS ) in lung of the rat with different size liver transplantation and it's significance. Methods The animal models of whole graft, 50% size and 30% size graft liver transplantation were established and the animals were sacrificed at 0.5-, 2-, 6- and 24h time points to harvest the lung specimens. The iNOS gene expression was detected by reverse-transcribed polymerase chain reaction (RT-PCR) and Western blot. The histopathological examination was also examined. Results (1) The iNOS mRNA levels were up-regulated in transplantation groups and the expression of the whole graft and 50% size groups was significantly higher than that of the 30% size group at 6-h time point (P<0.05);The iNOS protein expression was consistent with the mRNA expression. (2) The results of histopathologic examination shown that morphological structures were normal in the sham operation group. Obvious interstitial hyperemia and hemorrhage, neutrophils infiltration and pulmonary septal thicking were found in transplantation groups and more severe at 2-h time point. Conclusions The up-regulation of iNOS expression in lung may be involved in the lung injury after liver transplantation and it's expression may be correlatied with the size of grafts.
