氟比洛芬酯联合芬太尼在游离皮瓣移植术后镇痛的应用

目的:观察氟比洛芬酯联合芬太尼在游离皮瓣移植术后镇痛的效果。方法:选择ASAⅠ～Ⅱ级行游离皮瓣移植术病例60例,随机分成K组(氟比洛芬酯联合芬太尼)和F组(芬太尼组),每组30例。K组于手术结束前15 min静脉推注氟比洛芬酯50 mg,F组静脉推注0.9 NaCl溶液4 ml,术后均采用芬太尼自控静脉镇痛(PCIA)。记录术后2、6、12和24 h两组VAS评分和Ramsay评分,术后24 h两组PCA按压次数,芬太尼的用量及不良反应。结果:K组术后2、6、12 h VAS评分和Ramsay评分,术后24 h PCA按压次数,芬太尼用量均明显低于F组(P<0.05)。两组不良反应发生率的差异有统计学意义(P<0.05)。结论:氟比洛芬酯联合芬太尼用于游离皮瓣移植术后静脉镇痛效果更好,能减少术后芬太尼用量及不良反应。

Analgesic effect of flurbiprofen axetil combined with fentanyl for patients undergoing surgery of free skin flap

Objective To investigate the analgesic effect of flurbiprofen axetil combined with fentanyl in patients undergoing the surgery of free skin flap.Method Total 60 cases undergoing the surgery of free skin flap were randomly divided into group K（flurbiprofen axetil combined with fentanyl） and group F（fentanyl） equally.Patients in group K received intravenous flurbiprofen axetil 50 mg and group F received intravenous NS 4 ml,15 min before the end of the operation.All patients received fentanyl for postoperative analgesia.The score of VAS and Ramsay were recorded at 2,6,12,and 24 h after operation.The total number of pressing PCA pump,the total consumption of fentanyl and adverse effects were recorded at 24 h after operation.Results Compared with group F,there were significant decreases in the score of VAS and Ramsay,fentanyl consumption and the total number of pressing PCA pump in group K（P0.05）.Adverse effects had significant difference between the two groups（P0.05）.Conclusion Intravenenous analgesia of flurbiprofen axetil combined with fentanyl offers better analgesic effect and is able to reduce fentanyl consumption in patients undergoing the surgery of free skin flap.
