剪切波超声弹性成像在乳腺增生性疾病诊断中的价值

摘    要:目的:探讨Elastography Point Quantification (Elast PQ )剪切波弹性成像技术检测乳腺增生病人乳腺硬度的可行性。方法应用Elast PQ技术,对90例乳腺增生患者以乳头为中心分象限进行乳腺弹性模量值检测,分析不同象限测量的影响,获得参考值范围;并与传统应力式弹性成像评分进行对照。结果同一患者,外下象限与乳头区测得弹性模量值显著低于其他象限;内上象限、内下象限、外上象限测量值之间无统计学差异;Elast PQ测得的弹性模量值按应力式弹性超声对乳腺硬度评分分组,组间差距有明显统计学意义。结论 Elast PQ技术评估乳腺硬度具有可行性,综合多种因素考虑以外上象限弹性模量值代表性较好。

Abstract:Objective To discuss the feasibility of elastography point quantification (Elast PQ) imaging technology in de‐tecting breast hardness of patients with mammary hyperplasia .Methods Taking nipple as a center to divide into quad‐rants ,we used Elast PQ technique to determine the breast elastic modulus of 90 patients with mammary hyperplasia ,ana‐lyzed the effects of different quadrant measurements ,got the range of the reference value ,and then compared with elastic modulus by the traditional stress elastic imaging .Results For the same patient ,the elastic modulus values measured un‐der nipple and lower outer quadrant were significantly lower than those of the other quadrants .There were no statistic differences in upper inner quadrant ,lower inner quadrant ,and upper outer quadrant .If the elastic modulus value quantita‐tively measured by Elast PQ was divided into different groups according to the stress type elastic imaging score ,the breast hardness ,in different groups had statistically significant difference .Conclusion Elast PQ technique for assessing breast hardness is feasibility .Through a multi‐factor consideration ,the elastic modulus value of the upper outer quadrant is more representative .
