高流量阴茎异常勃起介入治疗3例的护理体会

目的探讨高流量阴茎异常勃起拟行介入手术围术期的护理措施,以减少手术并发症,促进患者康复.方法对3例外伤后导致的高流量阴茎异常勃起患者实施术前心理护理、常规检查、护患沟通、常规术前准备和术后体位护理、阴茎勃起及皮肤颜色观察、生命体征观察、术后心理护理及出院指导.结果 3例术后异常勃起完全消退均未发生并发症,定期随访1 a.栓塞2周左右后夜间有晨勃,但勃起硬度较以前下降,3个月后恢复性生活,射精后阴茎能够完全痿软,但性生活持续时间缩短,性交频率减少.结论高流量阴茎异常勃起行动脉导管进行超选择性阴茎海绵体动脉明胶海绵栓塞是最为安全有效的方法,且疗效确切,术期实施有效的护理措施,可提高介入手术成功率,预防并发症.

Interventional Therapy for High-flow Priapism :the Nursing Experience in Three Patients

Objective To discuss the perioperative nursing measures for interventional treatment of high-flow priapism and reduce complications and promote patient recovery.Methods A series of nursing measures were carried out in 3 patients who suffered from priapism caused by injury and received interventional therapy.The nursing measures included preoperative psychological nursing,routine preoperative examination,nurse-patient communication,routine preoperative preparation and postoperative care and posture,penile erection and skin color observation,observation of vital signs,postoperative psychosocial care and the health guidance at the time of discharge.Results Priapism was disappeared in the 3 patients without complications,the 3 patients were regularly followed up for 1 year.There was morning erection at night 2 weeks after interventional treatment,but erection hardness decreased.Sexual life of the patients was resumed 3 months after interventional treatment,penis was fully soft after ejaculation,but the sex life duration and frequency of sexual intercourse decreased.Conclusion Superselective arterial embolization with gelatin sponge in cavernous body of penis is the most safe and effective treatment for high-flow priapism.The preoperative preparation and postoperative care nursing measures can improve the success rate of surgical intervention,and prevent the incidence of complications.
