左旋卡尼汀抗氧化抗心力衰竭的临床研究

目的　探讨左旋卡尼汀 (L CN)治疗充血性心力衰竭 (CHF)的临床疗效及抗氧化作用。方法　CHF患者 92例 ,分为L CN组( 4 6例 )和对照组( 4 6例 )。对照组采用常规药物治疗 ,L CN组常规药物加L CN治疗 ,采用比色法检测治疗前后患者血清中谷胱甘肽 (GSH)、超氧化物歧化酶 (SOD)及丙二醛 (MDA)浓度 ,并观察心功能改变情况。结果　心衰患者治疗后血清中MDA浓度较治疗前明显下降 (P <0 .0 1 ) ,GSH ,SOD浓度较治疗前明显上升 (P <0 .0 1 ) ,L CN组MDA浓度在治疗后显著低于对照组 (P <0 .0 5 ) ,GSH ,SOD浓度在治疗后显著高于对照组 (P <0 .0 5 ) ;治疗后两组心功能较治疗前明显改善 ,L CN组心功能改善程度较对照组更明显 ,L CN组治疗显效率及总有效率均高于对照组 (P <0 .0 5 )。结论　L CN具有抗脂质过氧化物作用 ,有助于改善CHF患者的心功能

Clinical Study of Anti-congestive Heart Failure and Anti-lipid Peroxidation of L-carnitine

Objective To investigate the anti lipid peoxidation and improving the heart function effects of L carnitine (L CN)in patients with congestive heart failure (CHF).Methods 92 patients with CHF were divided into control group and L CN group (46 patients, respectively). The control group received routine medication: digitalis, diuretics, vasodilator, ACEI and B-blocker, while the L CN group were given intravenously L carnitine 3.0g for 10 days. The blood samples for assay of serum levels of MDA GSH SODwere taken previous to and after treatment. In addition, the changes of cardiac function of the patients were observed. Results The levels of MDA significantly decreased after L CN treatment compared with that in control group and before treatment ( P <0.05 or 0.01), the levels of GSH, SOD significantly increased after L CN treatment compared with that in control group and before treatment ( P <0.05 or 0.01), and cardiac function of CHF patients after L CN treatment were increased with that in control group ( P <0.05). Conclusion L CN may improving cardiac function in patients with CHF and playing an important role in anti lipid peroxidation.
