降钙素原及超敏C反应蛋白诊断新生儿败血症的临床价值

摘    要:目的:探讨降钙素原(PCT)及超敏 C 反应蛋白(hs-CRP)诊断新生儿败血症的临床价值。方法选取2012年2月-2013年6月在我院进行治疗的新生儿败血症患儿56例,设为观察组;同时选取同期在我院妇产科出生的健康新生儿56例,设为对照组。采用化学发光法检测 PCT,免疫比浊法检测 hs-CRP 水平,比较两组患儿血清PCT 值和 hs-CRP 水平,并比较血清 PCT 和 hs-CRP 的灵敏度、特异性、准确性三方面,分析其诊断新生儿败血症的临床价值。结果观察组 PCT 值和 hs-CRP 水平均远高于对照组,且观察组的 PCT≥0.5 ng/ ml 为阳性,hs-CRP 水平大于8.0 mg/ l 不在正常值之内,差异有统计学意义(P 〈0.05);血清 PCT 的灵敏度、特异性以及准确性均高于血清 hs-CRP 水平的检测,差异具有统计学意义(P 〈0.05)。结论降钙素原及超敏 C 反应蛋白都有助于新生儿败血症的诊断,且降钙素原优于超敏 C 反应蛋白的诊断,联合使用有利于提高诊断准确率。

Abstract:ObJective:To investigate the clinical value of procalcitonin and C-reactive protein in the diagnosis of neo-natal sepsis. Methods:The 56 cases with neonatal sepsis were selected as observe group during February 2012 to June 2013 in our hospital,and 56 healthy newborns in the same period in our hospital were selected as the control group. By chemilu-minescence detection of procalcitonin(PCT),turbidimetric immunoassay detection sensitivity C-reactive protein(hs-CRP) levels ,serum PCT values and hsCRP levels were compared to analyze the clinical value of the diagnosis of neonatal sepsis by comparing the sensitivity of serum PCT and hsCRP,specificity,accuracy. Results:The PCT values and hsCRP levels were much higher,and the observation group PCT ≥0. 5ng/ ml as positive,hsCRP levels greater than 8. 0mg/ L is not with-in the normal value,the difference was statistically significant(P 〈 0. 05);serum PCT sensitivity,specificity,and accura-cy of serum hsCRP levels were higher than the detection of a statistically significant difference(P 〈 0. 05). Conclusion:Procalcitonin and C-reactive protein in diagnosis contributes to the diagnosis of neonatal sepsis and procalcitonin superior sensitivity C-reactive protein in the diagnosis,and the combination will help improve diagnostic accuracy,which is worthy of clinical application.
