初步探讨TCD对中枢性眩晕患者的诊断价值

目的探讨经颅多普勒超声（TCD）对中枢性眩晕患者前、后循环血流变化的检测,明确其诊断价值。方法采用德国DWL TCD超声仪,对随机抽取的208例中枢性眩晕患者进行大脑中动脉（MCA）、大脑前动脉（ACA）、椎动脉（VA）、基底动脉（BA）检测,观察其收缩峰的平均速度（Vm）的变化,分析其年龄差异与前、后循环中各指标的变化水平。结果中枢性眩晕患者血流速度异常以后循环异常为主,高年龄组血流速度异常尤为明显。结论 TCD具有简便、安全、准确、重复性好等特点,能够为眩晕患者颅内血流情况提供动态观察指标,为临床诊断、治疗提供帮助,对眩晕患者的筛查有重要价值,可成为眩晕患者的常规检查。

The value of TCD in diagnosis of patients with central vertigo

Objective： To study of transcranial Doppler ultrasound（ TCD） on the centralvertigo patients with anterior and posterior circulation blood flow detection and diagnosis value. Methods： Using the German DWL TCD ultrasonography for middle cerebral artery,208 cases of central vertigo were randomly selected to（ MCA）,anterior cerebral artery（ ACA）,vertebral artery（ VA）,basilar artery（ BA） detection,and the average speed of systolic peak（ Vm） changes,changes of level of each index before and after in the circulation and its age difference were analyzed. Results： The detection of central vertigo patients with abnormal blood flow velocity after the circulation anomaly,the high age group velocity anomaly was obvious. Conclusion： TCD is a simple,safe,accurate,reproducible,which can provide dynamic observation for the vertigo patients with intracranial blood flow,help clinical diagnosis,treatment,has important value for the screening of patients with vertigo. It can be a routine examination of patients with vertigo.
