吻合口间断全层加强缝合预防食管癌贲门癌术后吻合口瘘

目的：探讨食管癌手术中一种进一步降低吻合口瘘的吻合口处理方法方法对112例中下段食管癌及贲门癌患者在全麻下行经左胸切除肿瘤，胃食管端侧机械吻合术，吻合口间断全层加强缝合结果本组发生吻合口瘘一例，无吻合口出血、胸腔胃坏死穿孔及胸内感染，无死亡，发生术后吻合口狭窄18例（16%）结论机械吻合后吻合口全层加强缝合，能有效降低食管癌吻合口瘘的发生，但可能增加吻合口狭窄发生率。

One-layer interrupted sutures after mechanical anastomosis to eliminate anastomotic leak in the surgery of Oesophageal and gastric cardia cancer

Objective：To evaluate a anastomotic technique of eliminating anastomotic leak after mechanical anastomosis in the surgery of Oesophageal and gastric cardia cancer. Methods：the clinical data of 112 patients after transthoracic resec-tion for carcinoma of the esophagus and gastric cardia were analyzed retrospectively. One-layer interrupted sutures were made after end to end mechanical anastomosis. Results：One anastomotic leakage was identified in the 112 patients,there was no anastomotic bleeding,intrathoracic gastric perforation and all these patients survived. eighteen patients（16% ）de-veloped a benign esophageal stricture. Conclusion：One-layer interrupted sutures after mechanical anastomosis can prevent anastomotic leakage but increase the strictures. Dilatation is a safe and effective treatment.
