事件相关电位P300对系统性红斑狼疮患者认知功能障碍的临床研究

目的 观察不同认知功能评分的系统性红斑狼疮(SLE)患者之间及其与正常对照之间事件相关电位(ERPs)P300波的潜伏期和波幅有无差别,为SLE患者认知功能障碍的早期诊断提供一种客观、可靠方法.方法 采用丹麦Key point诱发电位仪,听觉odball刺激序列刺激,对60例SLE患者及同期60例健康受试者进行P300波检测,同时对SLE患者采用改良的简易精神状态检查量表(MMMSE)进行测验,比较不同认知功能评分的SLE患者之间及其与正常对照者之间P300波潜伏期和波幅的差异.结果 SLE患者按MMMSE评分分为A(>85分)、B(75～85分)、C(＜75分)3组,每组患者P300波的潜伏期分别为(340.80±31.16)ms、(358.40±36.16)ms、(388.70±44.26)ms,波幅分别为(8.4±0.5)μV,(7.2±0.6)μV,(6.1±0.6)μV;健康受试者(对照组)潜伏期为(318.80±21.16)ms,波幅为(8.7±0.8)μV.SLE B、C两组患者P300波的潜伏期和波幅与对照组比较差异有统计学意义(P＜0.01),而SLE A组与对照组比较,P300波的波幅差异无统计学意义(P＞0.05),潜伏期延长(P＜0.01).结论 P300波可反映SLE患者的认知水平,较MMMSE评分更为客观,可作为评价SLE患者认知功能的一项有价值且无创的新指标.

Clinical research of event-related potential P300 on systemic lupus erythematosus patients with cognitive dysfunction
