无脊髓损伤的颈椎骨折脱位临床研究

摘    要:无脊髓损伤的骨折脱位是颈椎损伤一种特殊类型。本文报告28例,其中4例新鲜骨折脱位,24例为陈旧性损伤。屈曲性损伤27例,垂直压缩外力致伤1例。作者对X线片的非损伤和损伤节段椎管矢状径作了测量,结果表明,二者椎管矢状径均在15mm以上,无显著差异(P>0.05)。作者认为无脊髓损伤原因可能与下列因素有关:屈曲性颈椎骨折脱位合并椎弓骨折及分离移位,后部韧带结构撕裂并分离使间隙扩大等,只导致椎管有限的变形,而矢状径并不缩小。

Abstract:The cervical spine fracture and dislocation without spinal cord injury is a special type of cervcial injury. Of 28 cases reported, 27 cases were caused by flextion force and 1 case by vertical compression force. The sagital diameter of cervical canal in the injuried segments and normal segments on x -ray film were both more than 15 mm without significant difference (p>0. 05). The author attributed the safty of this kind of injury to the following factors. Flexion fracture and dislocation of the cervical vertebrae and neural arch and tear of the posterior ligament complex with separation caused the deformity of cervical canal but did not caused the decreasing of the sagital diameter of cervical canal at the same time.
