尘肺患者全肺大容量灌洗麻醉处理

目的探讨双腔支气管静脉复合麻醉在全肺大容量灌洗中的应用。方法60例0^＋～Ⅱ期尘肺患者在双腔支气管插管静脉复合全麻下行大容量全肺灌洗术。术中左侧单肺通气，右侧用温生理盐水（37℃）灌洗，1周后同法行左肺灌洗术。结果60例双腔支气管静脉麻醉、灌洗顺利，监测生命征平稳，PaO2和SpO2在单肺通气至灌洗结束（术后）均明显升高（P〈0．05），术后自觉症状均较前明显好转。结论尘肺患者进行大容量全肺灌洗术采用双腔支气管静脉复合麻醉，只要麻醉操作、管理正确，是安全有效的。

To deal with pneumoconiosis patients with a large number of whole lung lavage anesthesia

Objective Double-lumen endobronchial intravenous anesthesia in the whole lung lavage in high-volume applications. Methods 60 cases of 0^＋- Ⅱ period of pneumoeoniosis in the double-lumen endobronchial intubation in patients with venous general anesthesia composite large-capacity whole lung lavage.The left side during one-lung ventilation, the right to use warm saline （37℃） lavage, after 1 week left with the method of lavage. Results 60 cases of double-lumen endobronchial intravenous anesthesia, irrigation well, life-sign monitoring smooth, PaO2 and SpO2 to one-lung ventilation in end of irrigation （after） are significantly higher （P〈0.05）,postoperative symptoms are a marked improvement over the previous. Conclusion Pneumoconiosis were large capacity whole lung lavage using double-lumen endobronchial intravenous anesthesia, as long as the narcotic operation, management right, is safe and effective.
