AO钢板内固定结合小夹板外固定治疗股骨中下段骨折

目的：通过对ＡＯ钢板内固定结合小夹板外固定治疗股骨中下段骨折病例的回顾性分析，探讨一种能防止钢板松动、断裂，减少膝关节功能障碍及再骨折发生的简单而有效的方法。方法：随机抽取１９９３～１９９８ 年行单纯ＡＯ钢板内固定病例１５０ 例，１５０肢为对照组；ＡＯ钢板内固定结合小夹板外固定病例１９５例，１９５ 肢为观察组；术后６ 个月至５年都采用Ｘ光片复查、患肢负重行走及患肢膝屈伸功能测定。结果：对照组钢板松动、断裂率为１１．３３％ ，观察组松动、断裂率为３．０８％ ，统计学上有非常显著差异（Ｐ＜ ０．０１）；对照组膝关节功能优良率为７８．００％ ，观察组为８９．７４％ ，统计学上有非常显著差异（Ｐ＜ ０．０１）；对照组再骨折率为８．００％ ，观察组再骨折率为０。结论：ＡＯ钢板内固定结合小夹板外固定可以降低ＡＯ钢板松动、断裂及再骨折的发生率，提高患膝关节功能恢复的优良率。

Treatment for Fracture of Middle lower Part of Femur with AO Steel Plate Internal Fixation Combined with Small Splint External Fixation

Purpose:To probe into a simple and effective method to prevent looseness,breaking,decrease of the dysfunctiion of knee joint and refracture through retrospective analysis of fracture cases with AO steel plate internal fixation combined with small splint external fixation for fracture of middle lower part of femur.Method:To take randomly 150 case(150 limbs) with simple AO steel plate internal fixation in 1993~1998 as control group and take 195 cases(195 limbs) with AO steel plate internal fixation combined with small splint external fixation as observation group;after 6 months to 5 years of operation,to rexamine with X ray film and to determine the weight carrying walking and the flexation and extention of knee joint of wounded limbs.Results:The rate of looseness and breaking of steel plate in control group was 11.33%,those in observation group was 3.08%,showing significant difference( P <0 01).The rate of fineness of knee joint function in control group was 78.00%,and that in observation group was 89 24%,showing significant difference ( P <0 01).The rate of refracture in control group was 8.00% and that in observation was 0.Conclusion:This method could decrease the incidence of looseness and breaking of AO steel plate,and refracture,and increase the rate of fineness of recovery of knee joint function.
