腹膜外筋膜内子宫全切术与传统子宫全切术临床效果的探讨

目的探讨腹膜外筋膜内子宫全切术和传统子宫全切术的临床效果及术后生活质量的差别.方法选择我院2003年8月-2004年8月间,年龄40-55岁,子宫增大如孕10-16周的子宫肌瘤患者,行子宫全切除的共280例.随机抽取行腹膜外筋膜内子宫全切术的40例,为研究组;行传统子宫全切术的40例,为对照组.并对两种术式的术中、术后效果及并发症进行比较.结果腹膜外筋膜内子宫全切术,具有出血量少,术后留置尿管时间短,肠功能恢复快,对性生活质量影响小,术后病率低并可避免宫颈残端癌的发生等优点.结论腹膜外筋膜内子宫全切术优于传统术式,值得推广应用.

Contract the clinical effects of the extra-peritoneum intrafascial total hysterectomy with the traditional total hysterectomy

Obstractive To explove the clinical effects and post-operative life quality of the extra-peritoneum intrafaseial total hysterectomy and to draw comparison with the traditional total hysterectomy. Methods totalling 280patients with leiomyoma of uterus who uterine enlarge to 10-16weeks preganey undergo total hystereetomy in hospital from August in 2003 to August in 2004, 40-55years old.The experimental group consisting of 40patients undergo the extra-peritoneum intrafascial total hysterectomy was selected randomly from 280patients,otherwise the traditional total hysterectomy was used in 40 patients in eontraposed group, and the operative result and post-operative complications were compared beween two groups. Results the extra-peritoneum intrafascial total hysterectomy has the advantages of bleed loss,short time retaining of urinary catheter, quick recovery for peristalsis, the influence on the sexual life were less,low morbidity rate after operation,and it prevents the happaning of stump cancer of cervix. Conclusions The extraperitoneal intrafascial total hysterectomy is superior to the the traditional total hysterectomy and is worth to be popularized.
