窒息新生儿血清IL-6和IL-8水平变化及其临床意义

摘    要:目的 探讨外周血白细胞分素-8(IL-8)在窒息新生儿中的变化及临床意义.方法 采用ELISA法检测42例不同程度窒息新生儿不同病期血清的IL-6、IL-8水平,并与正常足月新生儿脐血42例进行对照.结果 窒息新生儿第1天血清IL--6为57.6 ng/L(中位数,以下同),IL-8为353.3 ng/L,明显高于正常脐血对照组的2.0、92 ng/L(Z=4.564~5.856,P<0.01).重度窒息组患儿血清IL--6、IL-8分别是87、579.6 ng/L,分别高于轻度组(52.6、336.3 ng/L)(Z=2.228~2.702,P<0.05).血清IL-6、IL-8水平间呈显著正相关(α=0.59~0.72,P<0.01).窒息组患儿动态观察发现,病程第3天时血清IL-6(56.6 ng/L)、IL-8(320.6 ng/L)水平较第1天时(分别为57.6、353.3 ng/L)差异均无显著性(Z=0.314~1.372,P<0.05);第7天时血清IL-6(14.6 ng/L)、IL-8(238.4 ng/L)水平均明显低于第1天和第3天时(Z=2.455,P<0.05,Z=3.622,P<0.01).结论 窒息新生儿血清中IL-6、IL-8水平明显增高,在新生儿窒息发病机制中起重要作用.

Abstract:Objective To explore the change of IL-6 and IL8 in asphyxia newborn and clinical significance.Methods Compared with the control group,ELISA method was used to determine the degree of IL-6 and IL8 in different asphyxia newborn and in different coures of disease.Results In asphyxia newborn group serum IL-6(57.6 ng/L)and IL-8(353.3 ng/L)were higher than the control group(2.0,92 ng/L)(Z=4.564~5.856,P<0.01)in the first day,heavy asphyxia group serum IL-6(87 ng/L)and IL-8(579.6 ng/L)were higher than light asphyxia group(52.6,336.3 ng/L)(Z=2.228~2.702,P<0.05)and had obviously positive correlation(a=0.59~0.72,P<0.01).Serum IL-6(56.6 ng/L) and IL-8(320.6 ng/L) did not have obviously difference compared with the first day on the third day(Z=0.314~1.372,P<0.05);serum IL-6(14.6 ng/L) and IL-8(238.4 ng/L) were lower than aboues on the seventh day(Z=2.455,P<0.05,Z=3.622,P<0.01).Conclusion Serum IL-6 and IL-8 degree improved apparently in asphyxia newborn which had important value in newborn asphyxia pathogenesis.
