显微手术治疗外侧裂胶质瘤的探讨

目的 探讨外侧裂胶质瘤的临床特点和手术疗效.方法 回顾性分析2004年6月至2007年12月在我科治疗的有完整病案记录、随访资料和明确病理诊断的13例外侧裂胶质瘤患者的资料.结果 术中肿瘤全切除7例,大部分切除4例,部分切除2例.术后两个月死亡1例.结论 对于外侧裂区脑胶质瘤手术治疗可选择显微镜下翼点入路分离保护侧裂血管,术中在镜下仔细区分肿瘤和正常额、颞叶界面,尽力全切肿瘤,是提高手术疗效、减少并发症及复发的良好保证.

Microsurgical treatment for gliomas located in sylvian fissure area

Objective To study the clinical features and operation technique related to treatment of the gliomas located in sylvian fissure area. Methods Retrospectively summarized the clinical data including the intact medical records, integrate following-up data and pathological data in 13 cases with sylvian fissure gliomas from Jun. 2004 to Dec. 2007. Results Under a microscope, 7 cases underwent the total removal of glioma, 4 cases received the subtotal removal, one died after 2 months. Conclusion The pterional approach is well suit-able for reseciton of the gliomas located in sylvian fissure area by microneurosurgey. The curative effect of surgery on the sylvian fissure gliomas can be improved by preservation of the important artery, vein and brain functional areas. There are fewer complications and recurrence.
