剖宫产术后持续硬膜外镇痛的应用观察

目的:观察剖宫产术后持续硬膜外镇痛对产妇的影响.方法:对80例剖宫产产妇术后采用持续硬膜外镇痛(A组)和度非合剂镇痛(B组),观察两组的镇痛效果、产后出血量、子宫复旧情况、胃肠功能恢复、下床活动时间、哺乳情况、胃肠道及泌尿道反应.结果:A组在镇痛效果上明显优于B组,下床时间及哺乳时间均早于B组,并有较轻的胃肠道反应(P＜0.001～P＜0.05);而产后出血量、子宫复旧、排尿及肠蠕动恢复上,两组差异无显著性(P＞0.05).结论:持续硬膜外镇痛经济简便,镇痛效果好,可促进母乳喂养,减轻剖宫产产妇术后疼痛,无明显副作用,有推广应用价值.

Observation of postoperation epidural analgesia after cesarean section

Objective:To observe the pain relief effect of continuous epidural anesthesia on the post cesarean section women.Methods:Forty one pregnant women were administered continuous epidural anesthesia after cesarean section with tramadol and bupivacaine(A group).Thirty nine pregnant women received pethidine and phenergan(B group).The degree of pain,side effects and postpartum hemorrhage were studied in two groups respectively.Results:The pain relief effect in the A group was much better than that in the B group( P <0.05),the time of out of bed activity,the time and quantity of lactation and the time of miction were also shorter than the B group( P <0.001).There were no significant differences in the incidences of postpartum hemorrhage,the involution of uterus,or gastrointestinal function restoration( P >0.05).Conclusions:The epidural anesthesia is safe and economical to post cesarean section patients and there is no obvious side effect.We suggest that it should be performed in the medical units under optimal conditions.
