依帕司他对糖尿病性胃轻瘫和自主神经病变的影响

摘    要:目的:观察依帕司他对糖尿病性胃轻瘫(DGP)和自主神经病变的影响。方法:16例DGP病人,其中男性10例,女性6例,年龄(51±s 13)a,糖尿病病程(12±5)a。均给予依帕司他50 mg, po,tid,共12 wk。观察服药前后胃电图(EGG)和心率变异性(HRV)的变化,并记录病人症状的积分情况。结果:依帕司他治疗后,胃电主频率比(PDF)增加(0.8±0.8)cpm,胃电主功率比(PDP)增强(83±32)μV,HRV各指标中低频范围内的功率(LF)降低(38±16)ms2,LF/高频范围内的功率降低1.8±0.4,余指标均增高(均P<0.01)。病人大多数自觉症状减轻或消失,不良反应轻微。结论:依帕司他具有促胃动力和改善交感、迷走神经平衡失调的作用,是治疗DGP安全、有效的药物。

Abstract:AIM:To observe the effects of epalrestat in treating diabetic gastroparesis (DGP) and auto-nomic neuropathy. METHODS; Sixteen patients with DGP (M10, F6, age (51±s 13) a), diabetic courses (12±5) a were selected. All patients were treated with epalrestat 50 mg, po, tid for 12 wk. Heart rate variability (HRV) and electrogastrogram (EGG) were measured before and after treatment, and therapeutic effect on clinical syndromes was assessed. RESULTS:EGG were significantly changed after treatment mainly manifested as increase of proportional dominant frequency (PDF) for (0.8±0.8) cpm and strengthening of proportional dominant amplitude (PDP) for (83±32) |xV. The low frequency (LF) and the ratio of LF/HF were decreased by (38±16) ms2 and 1.8±0.4, and the other parameters of HRV were increased. All parameters were improved significantly after treatment (P < 0.01) . Symptoms of major patients were improved, with rare adverse reactions. CONCLUSION:Epalrestat can enhance gastric motility and improve dysfunction of the regulation of vagal and sympathetic nerves system. It is safe and effective in treatment of DGP.
