摘    要:目的:探讨常规MRI联合扩散加权成像(DWI)对鉴别侵袭性和非侵袭性垂体瘤的价值。方法:回顾性分析35例经手术病理证实的垂体瘤患者的MRI及DWI影像资料,将非侵袭性(6例)和侵袭性垂体瘤(29例)分成A组和B组,选用Fisher检验对两组某些MRI表现的例数进行比较,而对肿瘤大小和ADC值比较用t检验。结果:肿瘤的发生部位、周围侵犯、肿瘤形状、大小及ADC值等MRI特点的比较,A组与B组差异均具有统计学意义(P〈0.005),当应用ADC值0.903×10-3mm2/s作为鉴别两组肿瘤的指标时,诊断符合率达97%。结论:常规MRI联合DWI(包括ADC值)对鉴别诊断侵袭性与非侵袭性垂体瘤及其邻近结构的受累均有重要价值。

Value of conventional MRI in combination with diffusion weighted imaging in the differentiatial diagnosis of invasive from noninvasive pituitary adenoma

Abstract:Objective:To study the value of MRI including diffusion weighted imaging (DWI) in pituitary adenoma for distinguishing invasive from non-invasive adenoma.Methods:We retrospectively reviewed the MRI features (including DWI) of 35 cases with pathology proved pituitary adenoma,which were divided into non-invasive adenoma (Group A,6 cases) and invasive adenoma (Group B,29 cases).Fisher exact test was used to compare the MRI findings of these two groups,additionally t test was used to compared the size and apparent diffusion coefficient (ADC) value of tumor between Group A and B. Results:Comparison of the location,adjacent structural invasion,shape,size and ADC value of tumors of Group A and B showing on MRI,there were significant statistic differences (P0.005).Using the ADC value of 0.903×10-3mm2/s (P0.001) as the parameter in the differential diagnosis of the 2 groups of pituitary adenoma,the diagnostic accuracy reached to 97%.Conclusion:Conventional MRI in combination with DWI (including ADC values) can be very valuable for differentiating invasive or non-invasive pituitary adenoma as well as the diagnosis of tumor involvement into the adjacent structures.
