丘脑出血80例分析

目的探讨丘脑出血的临床特点及预后。方法回顾总结80例丘脑出血患者的临床表现与预后。结果本组80例中,既往有高血压病史68例,入院时高血压者70例,意识障碍38例,瞳孔变化34例,双眼垂直运动障碍22例,感觉障碍66例,运动障碍68例,合并上消化道出血30例,死亡26例,病死率32.5%。结论高血压病和动脉硬化为本病的主要病因,意识障碍主要与脑脊液循环受阻或血肿压迫导致颅内压增高有关。双眼垂直运动障碍、瞳孔缩小是丘脑出血的特征性体征,运动障碍发生率高,其预后与出血量多少有关,合并上消化道出血预后差。

Analysis of 80 cases of thalamic hemorrhage

【Objective】To investigate the clinical features of thalamic hemorrhage and prognosis.【Methods】 Summarized 80 cases of thalamic hemorrhage in patients with clinical manifestations and prognosis.【Results】in 80 cases,68 patients with past history of hypertension,high blood pressure on admission were 70 cases,38 cases of disturbance of consciousness,pupillary changes in 34 cases,22 cases of binocular vertical movement disorders,sensory disturbances 66 cases,68 cases of movement disorders,30 cases with upper gastrointestinal bleeding and death in 26 cases,mortality 32.5%.【Conclusions】Hypertension and atherosclerosis-based diseases major cause of disturbance of consciousness mainly with cerebrospinal fluid circulation obstruction or hematoma leading to increased intracranial pressure related to oppression.Binocular vertical movement disorders,pupillary narrowing is characteristic signs of thalamic hemorrhage,the high incidence of dyskinesia,the prognosis and the number of bleeding related to the merger of upper gastrointestinal bleeding and poor prognosis.
