高血压患者脉搏波传导速度与炎症介质的关系

目的探讨高血压患者脉搏波传导速度(PWV)与炎症介质(C-反应蛋白CRP、血浆纤维蛋白原FIB)的关系及PWV作为心血管危险预测因素的临床应用价值。方法将101例高血压患者按高血压不同水平分组观察,对照组为60例健康体检者。采用自动脉搏波速度测定仪,测定各组的颈动脉-股动脉PWV,取10个测值的均值作为最后的PWV,同时测定101例高血压患者和60例健康体检者的FIB、CRP。结果高血压患者组的PWV、CRP、FIB均显著高于正常对照组(P0.01);随着血压水平的升高,高血压患者的PWV、CRP、FIB也逐渐升高,高血压1级与3级间达极显著差异;随着高血压患者的PWV增加,CRP和FIB也逐渐升高;不同组别PWV均值与相应组别的CRP和FIB呈正相关,并且尤其与FIB呈显著相关。结论高血压患者的PWV、CRP、FIB随着血压水平的升高而逐渐升高;高血压患者的PWV与炎症指标(CRP、FIB)密切相关,作为心血管危险的独立预测因素有较好的临床应用价值。

Relationship between pulse wave velocity and mediators of Inflammation in patients with essential hypertension

【Objective】 To investigate the relationship between PWV and CRP,FIB in patients and the application of PWV-the prediction index of cardiovascular disease.【Methods】 As objects to be surveyed 101 patients with hypertension were recruited to be compared with them.Their PWV of carotid-femoral were measured ten times by automatic.PWV measurement device to get their average as the final PWV Besides,heir FIB,CRP were also measured.【Results】 1.The levels of PWV,CRP,FIB in the patients with essential hypertension were all significantly higher than that of the normal persons（p0.01）.2.Among the patients with hypertension,the levels of PWV,CRP,FIB increased gradually with the step-up of blood pressure,and the difference between hypertension grade 1 and grade 3 was significant（p0.05）.3.At the same time,level of PWV has positive correlation with that of CRP and FIB.【Conclusion】 The levels of PWV,CRP,FIB in the patients with essential hypertension are increased gradually with the step-up of blood pressure;CRP and FIB increase with the PWV,suggesting that the levels of PWV,CRP and FIB indicated the degree of disease of the patients with essential hypertension.This study indicates that the levels of PWV have great clinical value for cardiovascular disease.
