下肢静脉疾病造影诊断及误诊原因分析

目的探讨顺行性下肢静脉造影在下肢静脉疾病诊断上的临床价值及误诊原因。方法回顾性分析150例数字成像顺行性下肢静脉造影的影像学资料,将病人分为正常组(32侧)和异常组(118侧)。结果正常下肢静脉28侧;异常下肢静脉122侧,其中单纯性瓣膜关闭不全12侧,交通静脉瓣膜关闭不全16侧,原发性下肢深静脉瓣膜关闭不全38侧,继发性下肢深静脉瓣膜关闭不全22侧,下肢深静脉血栓形成18侧,先天性下肢静脉发育异常12侧,静脉瘤样病变4侧,Cockett综合症2例。结论顺行性下肢静脉造影在下肢静脉疾病诊断上方便、准确,具有独特的临床应用价值,采用正确的造影技术可以避免误诊,提高诊断的准确性。

The angiographic diagnosis of lower extremity venous disease and the analysis of misdiagnosis

【Objective】To investigate the clinical value of anterograde venous angiography in the diagnosis of venous diseases,and the causes of misdiagnosis.【Methods】We retrospectively analyzed 150 sides of the digital imaging anterograde venous angiography imaging studies.Patients were divided into normal group（32 sides） and abnormal（118 sides）.【Results】showed 28 normal venous sides;122 abnormal venous sides,of which simple valve regurgitation in 12 sides,traffic venous valve insufficiency in 16 sides,primary venous valve insufficiency in 38 sides,secondary deep venous valvular incomplete in 22 sides,deep vein thrombosis in 18 sides,congenital venous abnormalities in 12 sides,venous tumor-like lesions on 4 sides,Cockett syndrome on 2 sides.【Conclusion】Anterograde venous angiography is a convenient and accurate method in the diagnosis of venous diseases,and has a unique clinical value.To avoid misdiagnosis and improve the accuracy of diagnosis,the correct contrast technique is needed.
