大鼠脊髓损伤后尿动力学改变及P2X3表达的研究

摘    要:目的探讨大鼠不同平面脊髓损伤(SCI)造成神经源性膀胱后尿动力学,和P2X3的表达情况。方法成年健康SD雌性大鼠52只,将其随机分为正常组、骶上脊髓损伤组、骶下脊髓损伤组。采用脊横断法制备大鼠神经源性膀胱模型。四周后行尿流动力学检查,免疫组化法和Western blotting检测P2X3表达情况。结果骶上脊髓损伤组组逼尿肌漏尿点压较对照组、骶下脊髓损伤组组明显升高,骶下脊髓损伤组组逼尿肌漏尿点压较对照组显著降低;骶上脊髓损伤组组P2X3表达较对照组、骶下脊髓损伤组组明显升高,骶下脊髓损伤组组P2X2表达较对照组明显降低。结论尿流动力学检查为神经源性膀胱的早期诊断及选择个体化治疗方案提供了一种有效的检查手段。骶上脊髓损伤组组为逼尿肌反射亢进,骶下脊髓损伤组组为逼尿肌反射无力,膀胱逼尿肌P2X3表达增高可能是逼尿肌反射亢进机制之一。

Abstract:【Objective】 Study the Urodynamics change and P2X3 expression of neurogenic bladder caused by spinal cord injury at different levels.【Methods】 RandomLy divide 52 adult SD female rats into 3 groups: healthy group,sacral spinal cord injured group and under sacral spinal cord injured group;Employ transection of spinal cord to make the neurogenic bladder model for each group;Four weeks later,conduct urodynamic study,and measure the P2X3 expression by using immunohistochemisty and Western blotting.【Results】 sacral spinal cord injured group detrusor leak point pressure than the control group,under sacral spinal cord injured group was significantly higher,under sacral spinal cord injured group detrusor leak point pressure was significantly lower than the control group;P2X3 expression in sacral spinal cord injured group than in the control group,sacral spinal cord injured group was significantly higher,under sacral spinal cord injured group P2X3 expression significantly lower than the control group.【Conclusion】 Urodynamic study is very useful for the early diagnosia and individualized treatment of the neurogenic bladder after spinal cord injury.The sacral cord injury group,detrusor overactivity,the sacral spinal cord injury group,detrusor weakness,amplitude of bladder detrusor increased P2X3 expression may be one of the mechanisms of detrusor hyperreflexia.
