胃癌胃穿孔15例诊治体会

目的 探讨胃癌穿孔的临床特点，以指导临床诊治和提高疗效。方法 对本院15例胃癌穿孔病人采用不同手术方式治疗，其中5例行单纯穿孔修补术，1例在穿孔修补术后4周早期施行二期根治性手术，2例行穿孔修补术和胃空肠吻合术；8例行姑息性胃大部切除术；2例行胃癌根治术。结果 本组死亡5例。穿孔修补术，胃大部切除和根治性切除手术后平均生存期为8个月、18个月、27个月．结论 早期诊断，综合治疗，是降低死亡率，提高生存质量的重要因素。

Experiences in the treatment of perforated gastric cancer

Objective To investigate the clinical characteristics of perforated gastric cancer, guiding its clinical diagnosis and increasing its therapeutic efficacy. Methods Fifteen patients with perforated gastric cancer were treated by different operations, Five had simple repair Of gastric cancer perforation, 1 had repair Of perforation followed by secondary radical operation 4 weeks latter, 2 had repair Of perforation and gastrojejunos- tomy, 8 had palliative subtatol gastrectomy, 2 patients had radical gastrectomy for gastric cancer. Results Five patients died, the mean survival times fur surgical treatment patients were 8 months , 18 months and 27months, respectively. Conclusion Early diagnosis and combination therapy may reduce mortality and improve patientsquality of life.
