妊娠期急性阑尾炎的临床特征、诊断与治疗

目的探讨妊娠期急性阑尾炎的临床特征、诊断、治疗及预后。方法对我院2000年1月-2006年12月6年间收治的35例妊娠阑尾炎的临床资料进行回顾性分析。结果本组35例均治愈出院,其中14例睡手术切除阑尾,术后未发生伤口感染、流产及早产等并发症。术后病理诊断为单纯性阑尾炎5例,急性化脓性阑尾炎4例及急性坏疽性阑尾炎5例,21例行保守治疗,仅1例在保守治疗痊愈后2个月阑尾炎复发,经行阑尾切除术后治愈出院。结论妊娠期合并急性阑尾炎的患者,及时的诊断和治疗是处理本病的关键。对于诊断明确,症状、体征严重的患者,不论妊娠周期如何,手术治疗为首道选的治疗方法。

The clinical feature, diagnosis and treament of appendicitis during pregnancy

Objective To investigate the clinical feature,diagnosis,treatment and prognosis of acute appendicitis during pregnancy. Methods From January 2001 to December 2006,the clinical data of 35 cases with acute appendicitis during pregnancy were analyzed retrospectively. Results All cases were cured to discharge from hospital. Among them,14 patients underwent appendectomy,and no postoperative complication as wound infection,abortion and premature birth were found. Postoperative pathological diagnoses were simple appendicitis in 5 cases,acute suppurative appendicitis in 4 cases and acute gangrenous appendicitis in 5 cases. 21 cases underwent conservative treatment. Only 1 case was recurrence after healing of conservative treament for 2 months,and this patient was cured by appendectomy. Conclusion In time diagnosis and treatment are the keys to manage acute appendicitis during pregnancy. Operative treatment is the first choice for the patients with confimed diagnosis,severe symptoms with physical signs and any period of pregnancy.
