E-钙粘素和CD44V6表达与食管癌预后的关系

目的 探讨E-钙粘素（E-CD）、CD44V6表达与食管癌患者临床病理及预后的关系。方法 对58例食管鳞癌患者术后标本和25例正常食癌组织应用SP法检测E-CD和CD44V6的表达，对其中42例进行了术后3年随访。结果 E-CD阳性表达率与食管鳞癌患者癌细胞浸润深度、淋巴结转移显著有关（P〈0．01），与食管癌细胞分化程度、临床分期有关（P〈0．05），E-CD表达阳性组1、2和3年生存率分别为94．12％、88．24％和58．82‰阴性组分别为83．81％、56．00％和12．00％（P〈0．01）；CD44V6阳性表达率与食管鳞癌细胞分化程度无关（P〉0．05），与食管鳞癌患者细胞浸润深度、淋巴结转移显著有关（P〈0．01），与食管鳞癌患者临床分期有关（P〈0．05），CD44V6表达阳性存活率1、2和3年生存率分别为83．33％、60．00％和16．67％，阴性组分别为100％、91．67％和66．68％（P〈0．01）。结论 E-CD和CD44V6表达与食管癌转移、预后密切相关，且该两项指标表达方式相反；当E-CD阴性、CD44V6阳性时，可能说明患者预后差，有转移可能。

Relationship of E-cadherin and CD44V6 expression and prognosis in esophageal carcinoma

Objective] To study the of E-CD and CD44V6 expression with the clinical pathological features and prognosis in esophageal carcinoma. Methods] E-CD and CD44V6 expression were evaluated in the tissues of 58 human esophageal carcinomas and 25 normal esophageal mucosa by using immunohistochemical SP method. 42 cases were followed up for 3 years. Results] The expression of E-cadherin was significiantly related to the invasion depth and lymph node metastasis, cell differentiation degree and clinical stage of the tumors. (P <0.01, P <0.05). Survival rate of the first year, the second year, the third year were 94.12%, 88.24%, 58.82% respectively in positive E-cadherin expression, that were 83.81%, 56.00%, 12.00% in negative expression(P <0.01). The expression of CD44V6 was related to the invasion depth and lymph node metastasis, clinical stage of the tumors (P <0.01, P <0.05), but not related to the cell differentiation degree(P >0.05). Survival rate of the first year, the second year, the third year were 83.33%, 60.00%, 16.67% in positive CD44V6 expression, that are 100%, 91.67%, 66.68% in negative expression (P <0.01). Conclusions] E-CD and CD44V6 expression are correlated with metastasis and prognosis in esophageal tumors. The patients would have bad prognosis and metastasis when E-cadherin expression was positive and CD44V6 expression was negative.
