血清BNP、D-二聚体、PCT和IL-10联合检测对心力衰竭并发肺部感染的诊断作用

目的 探讨B型利钠肽(brain natriuretic peptide,BNP)、D-二聚体(D-Dimer,D-D)、降钙素原(procalcitonin,PCT)和白细胞介素-10(interleukin,IL-10)联合检测在心力衰竭并发肺部感染诊断中表达变化及意义.方法 选取2015年3月至2017年3月本院收治的心力衰竭并发肺部感染患者50例为观察一组,单纯心力衰竭患者50例为观察二组,单纯肺部感染患者50例为观察三组,选取本院同期健康志愿者50例为对照组.分别检测受试者血清中BNP、D-D、PCT和IL-10的水平并进行统计分析.结果 与对照组相比,观察一组、观察二组和观察三组患者BNP、D-D、PCT水平显著升高,IL-10水平显著下降,差异具有统计学意义(P＜0.05).观察一组患者BNP、D-D、PCT水平显著高于对照组、观察二组和观察三组,IL-10水平显著低于对照组、观察二组和观察三组,差异具有统计学意义(P＜0.05).与BNP、D-D、PCT和IL-10各项单独检出阳性率相比,四指标联合检测阳性率、灵敏度和特异性明显高于单独检出,差异具有统计学意义(P＜0.05).结论 心力衰竭并发肺部感染诊断患者血液中BNP、D-D、PCT和IL-10水平呈异常状态;且BNP、D-D、PCT和IL-10联合检测及实时监测对于心力衰竭并发肺部感染具有重要的临床意义.

Diagnostic Effects of Serum BNP,D-dimer,PCT and IL-10 on Heart Failure Complicated with Pulmonary Infection

Objective To investigate the effects of Brain natriuretic peptide (BNP),D-dimer (DD),procalcitonin (PCT) and interleukin-10 (Interleukin,IL-10) in the diagnosis of heart failure complicated with pulmonary infection and its significance.Methods A total of 50 patients with heart failure complicated with pulmonary infectionin our hospital from March 2015 to March 2017 were enrolled as group 1.50 cases of simple heart failure patients were enrolled as group 2,and 50 cases of pulmonary infection alone were selected as group 3.In addition,50 healthy patients were selected as the control group.The levels of BNP,D-D,PCT and IL-10 in serum were measured and analyzed.Results Compared with the control group,the levels of BNP,D-D and PCT in all the observation groups were significantly higher than those in the control group,and IL-10 content decreased significantly (P ＜ 0.05).The levels of BNP,D-D and PCT in group 1 patients were significantly higher than those in the control group,and group 2 and 3.The IL-10 levels in group 2 and 3 were significantly lower than those in the control group (P ＜ 0.05).Compared with the positive rates of BNP,D-D,PCT and IL-10,four factors combined detection positive rate,sensitivity and specificity were significantly higher than the single detection (P ＜ 0.05).Conclusion The expression of BNP,D-D,PCT and IL-10 in the blood of patients with heart failure complicated with pulmonary infection was abnormal.Also,BNP,D-D,PCT and IL-10 combined detection and real-time monitoring for heart failure complicated with pulmonary infection has important clinical significance.
