急性脑血管病甲状腺功能变化的动态观察

摘    要:目的观察脑血管病急性期甲状腺功能的变化情况,探讨其与病情程度及预后的关系.方法用化学发光法检测96例急性脑血管病病人发病第2天及第14天血清FT3、FT4、TSH水平,与40例正常对照组比较.结果脑血管病早期(第2天)FT3明显降低(P<0.001),而FT4、TSH明显增高(P<0.001、P<0.005),并且神经功能缺损评分值越高,这种变化就越明显;治疗2周后,病情较轻的病人以上3项指标逐渐恢复并接近正常,重症病人恢复缓慢.结论脑血管病急性期甲状腺功能异常,其变化情况与病情有关,动态观察其变化,对评估病情和预后有一定的价值.

Abstract:Objective To study ralations between the changes of thyroid function in patients with acute cerebrovascular disease and state of an illness and prognosis.Methods The thyroid's function of 96 patients suffering from cerebrovascular diseases was tested by chemiluminescence method on the second day and 14th day respectively after stroke,and compared with control group(n=40).Results The level of FT 3 was significantly decreased(P<0.001),but the levels of FT4 and TSH were obviously elevated(P<0.001,P<0.005)compared with control group in the patients with stroke on the second day.And the more of the scores of damage of neurofunction(DNF),the more obvious of the changes.After two wakes of treatment the three indexes were recovered to almost within the normal range in the milder patients but they were recovered slowly in the severe patients.Conclusion The thyroid function was abnormal in patients of the acute cerebrovascular disease. Observing the changes of thyroid function can help to assess state of an illness and prognosis.
