儿童闭合性肾损伤的诊治体会

目的 总结儿童闭合性肾损伤的特点及诊治经验. 方法 对比30例儿童与36例成人肾损伤的临床资料. 结果 儿童组重度肾损伤(AAST Ⅲ～Ⅴ级)发生率为50%(15例),高于成人组的31%(11例)(P<0.05),手术探查率儿童组为40%(12例),低于成人组的73%(P<0.05),儿童组伤肾切除率(7%)低于成人组(36%)(P<0.01). 结论 儿童易造成重度肾损伤,但其修复能力强,可适当限制手术探查及伤肾切除.

Diagnosis and treatment of blunt renal injuryin children

Objective To summarize the characteristics of blunt renal injury in children and re-port experiences in its diagnosis and treatment. Methods A comparison was done on medical data of 30 children and 36 adults with blunt renal injury. The renal injuries were classified according to the organ inju-ry scaling committee of the American Association for the Surgery of Trauma. Results The rate of severe renal injury in the children group was 50%, significantly higher than 31% in the adult group(P<0.05).Surgical exploration rate in the children group was 40%, lower than 73% in the adult group(P<0.05).In the meantime, the kidney conservation rate of 93% in children group was higher than 64% in adult group(P<0.01). Conclusions The kidney of children is susceptible to severe injury but has sound serf-repairing capability. Therefore, surgical interventions and nephrectomy should be considered cautiously.
