144例阑尾炎手术患者围手术期抗菌药物应用分析

目的：了解我院阑尾炎手术患者围手术期抗菌药物的使用情况,为临床合理用药提供参考。方法：回顾性调查并分析我院2008年普通外科阑尾炎住院手术患者病例144份,统计其抗菌药物的品种选择、联合用药、用药时间、不合理用药等情况。结果：144例阑尾炎手术患者抗菌药物使用率为100%,其中,预防性用药96.53%,且均为静脉滴注给药。抗菌药物应用率较高的前5位分别是奥硝唑（25.3%）、依替米星（10.1%）、头孢地嗪（9.1%）、五水头孢唑啉钠（7.8%）、甲硝唑（7.5%）。联合用药中,二联用药比例最大,占91.67%。结论：我院阑尾炎手术患者围手术期抗菌药物的临床应用存在用药时间长、选药档次高等不合理现象,因此要加强我院阑尾炎手术预防性使用抗菌药物的管理,进一步提高抗菌药物合理使用率。

Application analysis on perioperative antibiotics in 144 appendectomy patients

Objective：To analyze the perioperative use of antibiotics in patients undergoing appendectomy,and provide reference for rational drug use.Methods：A retrospective investigation on the perioperative use of antibiotics was conducted in 144 patients undergoing appendectomy in general surgical department in our hospital in 2008,and application of the antimicrobial agents was analyzed statistically in respect of drug varieties,drug combination,time of administration and irrational prophylactic antibacterial use,etc.Results：All cases had used antibiotics,96.53% of them were prophylactic administration by intravenous drip.The top 5 drugs in terms of use frequency were ornidazole（25.3%）,etimicin（10.1%）,cefodizime（9.1%）,cefazolin sodium pentahydrate（7.8%）,metronidazole（7.5%）.The combined use of two antibiotics accounted for the highest proportion（91.67%）.Conclusion：The irrational use of antibiotics in perioperative period of appendectomy in our hospital manifested as prolonged medication duration and high grade in the choice of antibiotics,etc.It is necessary to strengthen the control on the prophylactic use of antibiotics in appendectomy patients in our hospital so as to raise the rational use rate of antibiotics.
