辛伐他汀对急性冠脉综合征患者胰岛素敏感性的影响

目的观察辛伐他汀对急性冠脉综合征(ACS)患者胰岛素敏感性的影响。方法 40例急性冠脉综合征患者采用随机、单盲、对照的原则分为两组,每组20例。一组为常规治疗组(常规治疗,不服用任何调脂药物),另一组为辛伐他汀组(常规治疗加辛伐他汀20mg/d),治疗前和治疗4周后分别检测血清总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、甘油三酯(TG)、空腹血糖 (FBG)以及空腹胰岛素(FINS),并计算出胰岛素敏感指数(ISI)；同时选择健康体检者20名为健康对照 (正常对照)组。结果急性冠脉综合征患者与正常对照组比较,存在高胰岛素症及胰岛素敏感性降低。常规治疗组治疗4周后胰岛素敏感性未见明显变化(P＞0.05),加用辛伐他汀治疗4周后胰岛素敏感性有明显改善(P＜0.05),两组比较有显著性差异(P＜0.05)。结论急性冠脉综合征患者存在胰岛素抵抗,辛伐他汀可改善急性冠脉综合征患者胰岛素抵抗,提高其敏感性。

Influence of Simvastain on insulin sensitivity of patients with acute coronary syndrome

Objective To study the influence of Simvastain on insulin sensitivity (ISI) of patients with acute coronary syndrome. Methods The levels of fasting serum lipids (TC, TG, LDL-C, HDL-C), fasting blood glucose (FBG) and fasting blood insulin (FINS) were determined in 20 normal control and 40 cases of acute coronary syndrome. Then, 40 cases of acute coronary syndrome were divided into two groups by using a randomized, control and single blind method, Simvastain group(n=20) and routine treatment group (n=20). With a treatment course of 4 weeks for both groups, the above indices were determined again. Results The insulin sensitivity index (ISI) was lower in patients with acute coronary syndrome than those in the normal control (P＜0.05). Later 4 weeks after the treatment, ISI significantly increased in Simvastain group (P＜0.05), but ISI showed no significantly in routine treatment group (P＞0.05). Conclusion Insulin resistance exists in patients with acute coronary syndrome. Simvastain can improve the ISI of patients with acute coronary syndrome.
