腹腔镜辅助下经阴道子宫全切治疗子宫肌瘤的临床效果观察

目的探讨腹腔镜辅助下经阴道子宫全切治疗子宫肌瘤的临床效果。方法选择2010年1月~2013年9月本院收治的子宫肌瘤行手术子宫全切患者80例,按照随机数字表法分为观察组和对照组各40例,观察组采用腹腔镜辅助下经阴道子宫全切术,对照组实施单纯的经阴道子宫全切术,比较两组的手术时间、术中出血量及住院时间,并统计并发症发生率。结果观察组手术时间及住院时间均显著短于对照组（P〈0.05）,术中出血量少于对照组（P〈0.05）;术后伤口感染及术后严重疼痛的发生率均显著低于对照组（P〈0.05）,对所有患者随访1年,其中观察组术后性交痛的发生率显著低于对照组（P〈0.05）。结论腹腔镜辅助下阴式子宫全切术相对于单纯阴式子宫全切术,手术时间短,术后恢复快,并发症少,值得临床推广。

The clinical effect observation of laparoscopic assisted vaginal hysterectomy in treatment of uterine fibroids

Objective To investigate the clinical effects of laparoscopic assisted vaginal hysterectomy in the treatment of uterine fibroids. Methods 80 cases of patients with uterine fibroids treated in our hospital from January 2010 to September 2013 were selected and were randomly divided into observation group and control group,40 cases in each group,laparoscopic assisted vaginal hysterectomy was taken to the observation group,the control group were taken with simple vaginal hysterectomy,then operative time,intraoperative the amount of bleeding and length of hospital stay,and complications in the two groups were compared. Results The operative time and hospital stay of observation group was shorter than that of control group respectively（P〈0.05）,blood loss was less than that of control group（P〈0.05）;postoperative wound infections and severe postoperative pain was lower than that of control group respectively（P〈0.05）,all patients were followed up for one year,dyspareunia appear of the observation group was lower than that of control group（P〈0.05）. Conclusion Compared with simple vaginal hysterectomy,laparoscopic assisted vaginal hysterectomy has shorter surgery time and rapid postoperative recovery and fewer complications,so it is worthy of promotion in clinic.
