厄贝沙坦对糖尿病肾病患者的影响

目的探讨厄贝沙坦对糖尿病肾病（DN）患者血清骨桥蛋白（OPN）水平及尿白蛋白排泄率（UAER）的影响。方法选择2013年1月~2014年3月本院收治的DN患者98例,随机分为两组,每组为49例,常规治疗组予以糖尿病教育、糖尿病饮食、适当运动、皮下注射胰岛素等常规治疗,厄贝沙坦组在常规治疗基础上加用厄贝沙坦150 mg,1次/d。治疗12周,比较治疗前后两组患者血清OPN、UAER等指标水平的变化。结果治疗后,两组患者血清OPN水平、UAER均降低（P﹤0.05）;与常规治疗组比较,厄贝沙坦组血清OPN水平及UAER降低更明显（P﹤0.05）。结论厄贝沙坦可明显降低DN患者UAER及血清肌酐水平,从而保护肾功能,其机制可能与降低血清OPN水平有关。

Influence of irbesartan on patients with diabetic nephropathy

Objective To probe into the influence of irbesartan on the level of serum osteopontin（OPN）and urinary albumin excretion rate（UAER）in patients with diabetic nephropathy（DN）. Methods 98 patients with DN admitted into our hospital from January 2013 to March 2014 were selected and randomly divided into two groups,each group of 49 cases.Regular therapy of diabetes education,diabetic diet,appropriate exercise and subcutaneous injection of insulin were provided in conventional treatment group,on the basis of conventional treatment,irbesartan（150 mg,once per day）was added in irbesartan group.After treatment of 12 weeks,change of serum OPN and UAER before and after treatment in two groups were compared. Results After therapy,the levels of serum OPN and UAER both decreased（P 〈0.05）.Compared with conventional treatment group,the levels of serum OPN and UAER further decreased in irbesartan group（P〈0.05）.Conclusion Irbesartan can further decrease the levels of UAER and serum creatinine in DN patients so as to protect renal function.The mechanism may relate with the decreasing level of serum OPN.
