高危妊娠对母婴不良结局的影响

目的了解高危妊娠发生的基本情况及其对母婴不良结局的影响。方法随机选择福田辖区1926例产妇的母子保健手册及分娩登记本进行统计分析。结果1926例产妇中,A级5分以上的高危妊娠共有226例,发生率11.73%。226例高危妊娠中,A级66例,占29.20%;B级136例,占60.18%;C级24例,占10.62%。高危因素前三位排位及构成比依次为过期妊娠(15.19%)、剖宫产史(11.66%)、早产(11.31%)。随着高危级别的递增,胎婴儿不良结局发生率呈显著增高趋势。结论过期妊娠、剖宫产史、早产是高危妊娠多发因素,母婴不良结局尤其是胎婴儿不良结局的发生与高危妊娠密切相关,应加强围产保健宣教,加强孕期保健和高危妊娠监护,加强产时防治措施,以促使高危因素的转化和减少不良结局的发生。

THE AFFECT OF HIGH RISK PREGNANCY ON ILL OUTCOME OF MOTHER AND CHILD

Objective To investigate the basic situation of high risk pregnancy and its impact on ill outcomes. Methods Select 1926 lying-in women of Futian District at random, analyze the obstetrics clinical data by review the mother-baby notebooks and labour notebooks. Results Of all 1 926 cases, the prevalence of high risk pregnancy (score above 5A) was 11.73%, about 226 cases. Among these 226 cases, level A was accounting for 29.20% (66 cases), level B 60.18% (136 cases), and level C 10.62 (24 cases). The first three high risk factors were prolonged pregnancy (accounting for 15.19%), history of Cesarean section (11.66%), and premature birth (11.31%). The higher the level was, the more probability of the fetus or the newborn having ill outcome. Conclusion Prolonged pregnancy, history of Cesarean section and premature birth were the most common risk factors of high risk pregnancy. The ill pregnancy outcome especially the ill outcome of fetus and newborn has close connection with high risk pregnancy. We should strengthen the education of perinatal period care, strengthen the pregnancy health care and the monitor of high risk pregnancy, strengthen the monitor of labour, in order to urge high risk pregnancy transform and decrease the prevalence of ill outcome.
