64层螺旋CT检测颈动脉斑块和狭窄与前循环缺血性脑卒中的关系

摘    要:目的探讨前循环颅内、外段血管斑块和狭窄分布特点及其与前循环缺血性脑卒中的关系。方法选择80例前循环缺血性脑卒中患者为实验组,另选同期60例非缺血性脑卒中患者为对照组,入院1周内检测血脂、血尿酸及64层螺旋CT血管成像,检测双侧前循环颅内、外段斑块性质及血管狭窄程度。结果与对照组比较,实验组血管狭窄检出率明显升高(73.3%vs 91.3%,P<0.01),斑块发生率明显升高(55.0%vs 90.0%,P<0.01)。与颅外段血管中重度狭窄比较,颈动脉颅内段中重度狭窄明显增高(40.0%vs 60.0%,P<0.05)。结论颈动脉内的斑块形成及血管狭窄是前循环缺血性脑卒中发生的主要病因;颈动脉颅内段血管狭窄较颅外段常见,但颈动脉颅外段血管的不稳定斑块居多。

Role of 64-slice spiral CT in demonstrating the relation of carotid plaques and stenosis with anterior circulation ischemic stroke
Abstract:Objective To study the relation of extra-intracranial atherosclerotic plaques and stenosis with anterior circulation ischemic stroke.Methods Eighty patients with anterior circulation ischemic stroke served as an experimental group and 60patients without ischemic stroke served as a control group in this study.Their serum lipid and uric acid levels were measured within 1week after admission.Their bilateral extra-and intracranial atherosclerotic plaques and stenosis were detected by 64-slice spiral CT.Results 64-slice spiral CT showed that the incidence of stenosis and plaques was significantly higher in experimental group than in control group(91.3% vs 73.3%,P<0.01;90.0% vs 55.0%,P<0.01)and the incidence of extracranial carotid artery moderate stenosis was higher than that of intracranial carotid artery severe stenosis(60.0% vs 40.0%,P<0.05).Conclusion Carotid artery plaque formation and vascular stenosis are the major pathogenic factors for anterior circulation ischemic stroke.Intracranial carotid artery stenosis is more common than extracranial carotid artery stenosis.However,the number of unstale extracranial carotid artery plaques is greater than that of unstale intracranial carotid artery plaques.
