三种负荷试验对冠心病诊断的对比研究

目的 :以冠状动脉造影结果为标准 ,评价ATP负荷超声心动图试验 (ATPE)、多巴酚丁胺负荷超声心动图试验 (DOBE)和平板运动心电图试验 (EE)对冠心病的诊断价值。方法 :对 5 6例疑诊冠心病患者而行冠状动脉造影检查的患者 ,在同期内 ,即间隔 <4周 ,分别接受ATPE、DOBE和EE负荷试验 ,将冠状动脉造影结果分别与三项负荷试验结果对比分析。结果 :ATPE、DOBE和EE负荷试验诊断冠心病的敏感性分别为 :80 %、83%和 77% ;特异性分别为 :96 %、88%和 81%。检出冠心病的准确率分别为 88%、86 %、79%。结论 :ATPE、DOBE和EE三种负荷试验对冠心病的诊断有着较高的敏感性和特异性 ,费用低 ,重复性好 ,是比较安全、有效的检查方法 ,而选用何种检查要依据患者具体情况来确定。

Three kinds of stress methods in the diagnosis of coronary artery disease

Objective: To evaluate the diagnostic value of three kinds of methods: exercise electrocardiography(EE),dobutamine stress echocardiography(DOBE) and ATP stress echocardiography (ATPE) in coronary artery disease(CAD) according to coronary angiography. Methods: There are fifty-six patients who were examined with coronary angiography, EE, DOBE and ATPE. The date of coronary angiography were compared with the three kinds of methods. Results: The sensitivity in the diagnosis of CAD with the three kinds of methods were 77%,83% and 80%,respectively.The specificity were 81%,88% and 96%, respectively. The accuracy were 79%,86%,88%, respectively. Conclusions: The three kinds of methods had high sensitivity and specificity, and reached high diagnostic accuracy. They were lower cost and effective metheds.
