右美托咪定联合臂丛神经阻滞麻醉在小儿手切割伤手术麻醉中的应用效果

目的探究右美托咪定联合臂丛神经阻滞麻醉在小儿手切割伤手术麻醉中的应用效果。方法选取手切割伤手术患儿86例作为研究对象,按照随机数表将其分为对照组和观察组,各43例。对照组给予咪达唑仑联合臂丛神经阻滞麻醉,观察组给予右美托咪定联合臂丛神经阻滞麻醉。比较两组的麻醉效果。结果T₁、T₂、T₃时,两组MAP、HR较T₀时下降,且观察组显著低于对照组(P<0.05);T₄时,两组MAP、HR较T₁时显著上升,且观察组显著高于对照组(P<0.05)。观察组监测治疗室滞留时间、术后清醒时间、术后首次活动时间均显著短于对照组(P<0.05)。观察组总遗忘率显著低于对照组(P<0.05);两组患儿不良反应总发生率无显著差异(P>0.05)。术后3 h,两组NE、Cor水平均高于术前,但观察组低于对照组(P<0.05)。结论右美托咪定联合臂丛神经阻滞麻醉对手切割伤患儿术后具有一定的镇痛效果,但其遗忘作用不佳。

Application effect of dexmedetomidine combined with brachial plexus block anesthesia on pediatric hand cut operation

Objective To explore the application effect of dexmedetomidine combined with brachial plexus block anesthesia on pediatric hand cut operation.Methods Eighty-six children with hand cut operation were selected as the research objects and divided into control group and observation group according to the random number table,with 43 cases in each group.The control group was given midazolam combined with brachial plexus block anesthesia,and the observation group was given dexmedetomidine combined with brachial plexus block anesthesia.The anesthetic effects of the two groups were compared.Results At T₁,T₂ and T₃,MAP and HR of the two groups decreased compared with T₀,and those of the observation group were significantly lower than the control group(P<0.05);at T₄,MAP and HR of the two groups increased compared with T₁,and those of the observation group were significantly higher than the control group(P<0.05).The retention time of monitoring treatment room,postoperative wake-up time and the first postoperative activity time in the observation group were significantly shorter than those in the control group(P<0.05).The total forgetting rate of the observation group was significantly lower than that of the control group(P<0.05);there was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).At 3 h after operation,the levels of NE and Cor in both groups were higher than those before operation,but those in the observation group were lower than the control group(P<0.05).Conclusion Dexmedetomidine combined with brachial plexus block anesthesia has a certain anesthetic effect in children with hand cut injury,but its forgetting effect is not good.
