不同剂量盐酸右美托咪定在慢性鼻窦炎鼻息肉术中的麻醉效果及不良反应发生情况

目的分析慢性鼻窦炎鼻息肉术中选用不同剂量盐酸右美托咪定的麻醉效果和不良反应发生情况。方法选取2018年4月至2019年12月于我院接受手术治疗的80例慢性鼻窦炎鼻息肉患者作为研究对象,通过随机数字表法将其分为对照组和试验组,各40例。两组均在麻醉诱导前10 min输注负荷剂量(0.6μg/kg)盐酸右美托咪定,在此基础上,对照组给予0.3μg/kg维持剂量的盐酸右美托咪定,试验组给予0.6μg/kg维持剂量的盐酸右美托咪定。比较两组患者的心率、血压、拔管时间、镇静效果及不良反应发生情况。结果手术开始时及拔管时,试验组的心率、收缩压、舒张压均明显低于对照组,差异具有统计学意义(P<0.05)。两组的拔管时间比较,差异无统计学意义(P>0.05)。试验组的Ramsay镇静评分明显高于对照组,SAS评分明显低于对照组,差异具有统计学意义(P<0.05)。两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。结论慢性鼻窦炎鼻息肉术中选用0.6μg/kg维持剂量的盐酸右美托咪定,可以有效提升镇静效果,稳定患者的血压和心率,且安全性较高,值得临床推广应用。

Anesthetic effect and adverse reactions of different doses of dexmedetomidine hydrochloride in the operation of chronic sinusitis and nasal polyps

Objective To analyze the anesthetic effect and adverse reactions of different doses of dexmedetomidine hydrochloride in the operation of chronic sinusitis and nasal polyps.Methods A total of 80 patients with chronic sinusitis and nasal polyps who received surgical treatment in our hospital from April 2018 to December 2019 were selected as the research objects,and the patients were divided into control group and experimental group by random number table method,with 40 patients in each group.Both groups were given a load dose of dexmedetomidine hydrochloride(0.6μg/kg)at 10 min before induction of anesthesia.On this basis,the control group was given 0.3μg/kg maintenance dose of dexmedetomidine hydrochloride,and the experimental group was given 0.6μg/kg maintenance dose of dexmedetomidine hydrochloride.The heart rate,blood pressure,extubation time,sedative effect and adverse reactions of the two groups were compared.Results At the beginning of operation and extubation,the heart rate,systolic pressure and diastolic pressure of the experimental group were significantly lower than those of the control group,and the differences were statistically significant(P<0.05).There was no statistically significant difference in the extubation time between the two groups(P>0.05).Ramsay sedation score of the experimental group was significantly higher than that of the control group,SAS score was significantly lower than that of the control group,and the differences were statistically significant(P<0.05).There was no statistically significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion The 0.6μg/kg maintenance dose of dexmedetomidine hydrochloride used in the operation of chronic sinusitis and nasal polyps can effectively enhance the sedative effect,stabilize the blood pressure and heart rate of patients,and has high safety,which is worthy of clinical promotion and application.
