宫腔球囊填塞术与宫腔纱布填塞术治疗产后出血的效果比较

目的比较宫腔球囊填塞术与宫腔纱布填塞术治疗产后出血的效果。方法选取我院2018年1月至2020年1月收治的42例产后出血患者作为研究对象,按照治疗方式将其分为观察组(21例,宫腔球囊填塞术)和对照组(21例,宫腔纱布填塞术)。比较两组患者的治疗效果。结果观察组阴道分娩、剖宫产手术时间、术中出血量、术后24 h出血量及输血量均优于对照组(P<0.05);观察组阴道分娩的手术时间、术中出血量、术后24 h出血量及输血量均优于剖宫产(P<0.05)。观察组术后子宫收缩能力明显优于对照组,填塞物取出时间和住院时间均短于对照组(P<0.05)。观察组不良事件总发生率明显低于对照组,止血总成功率明显高于对照组(P<0.05)。结论采用宫腔球囊填塞术治疗产后出血比纱布填塞术更安全有效,尤其是在阴道分娩产后出血。宫腔球囊填塞术能有效减少术后出血量和输血量,患者恢复时间短,止血成功率高,且不良事件发生率低,值得临床推广。

Comparison of the effects of intrauterine balloon tamponade and intrauterine gauze tamponade in the treatment of postpartum hemorrhage

Objective To compare the effects of intrauterine balloon tamponade and intrauterine gauze tamponade in the treatment of postpartum hemorrhage.Methods A total of 42 patients with postpartum hemorrhage admitted in our hospital from January 2018 to January 2020 were selected as the research objects and divided into observation group(21 cases,intrauterine balloon tamponade)and control group(21 cases,intrauterine gauze tamponade)according to the treatment methods.The therapeutic effects of the two groups were compared.Results The operative time,intraoperative blood loss,blood loss within 24 h after operation and blood transfusion volume of vaginal delivery and cesarean section in the observation group were better than those in the control group(P<0.05);in the observation group,the operative time,intraoperative blood loss,blood loss within 24 h after operation and blood transfusion volume of vaginal delivery were better than those of cesarean section(P<0.05).The postoperative uterine contractility in the observation group was significantly better than that in the control group,and the time to remove the tamponade and hospital stay were shorter than those in the control group(P<0.05).The total incidence of adverse events in the observation group was significantly lower than that in the control group,and the total success rate of hemostasis was significantly higher than that in the control group(P<0.05).Conclusion Intrauterine balloon tamponade is safer and more effective than gauze tamponade in the treatment of postpartum hemorrhage,especially in vaginal delivery.Intrauterine balloon tamponade can effectively reduce the postoperative blood loss and blood transfusion volume,with short recovery time,high success rate of hemostasis,and low incidence of adverse events,which is worthy of clinical promotion.
