MRI findings of reversible splenial lesion syndrome

Objective To explore the magnetic resonance imaging(MRI)features of reversible splenial lesion syndrome(RESLES).Methods A total of 11 patients with RESLES diagnosed by clinical obserration and MRI imaging from January 2017 to June 2019 in our hospital were selected retrospectively.The etiology,clinical observation manifestations,MRI imaging manifestations,treatment and prognosis were analyzed.Results The etiology of 11 patients were different,8 cases were infection,including 1 case with hyponatremia,and another 3 cases with hypoglycemic encephalopathy.MRI showed isolated abnormal signal in the splenium of corpus callosum(SCC)in 9 patients,which was patchy,round or oval,distributed symmetrically in the middle,isointense or slightly hypointense on T₁WI,slightly hyperintense on T₂WI,slightly hyperintense on T₂FLAIR,hyperintense on DWI and hypointense on ADC.One patient had abnormal signal of SCC combined with symmetrical abnormal signal of posterior limb of bilateral internal capsule.One patient had abnormal signal of SCC combined with abnormal signal of left temporal lobe.Reexamining MRI,the abnormal signal of SCC disappeared completely in 11 patients.Conclusion There are many causes and complex clinical manifestations of RESLES,MRI images of RESLES are characteristic and its DWI shows high signal intensity of SCC,distributed symmetrically in the middle,with or without abnormal signal outside the corpus callosum,which disappears reversibly.MRI can objectively reflect the characteristics of lesions,and has important clinical value in the diagnosis,differential diagnosis and prognosis of RESLES.
