腹部淋巴管瘤的CT诊断

目的：探讨腹部淋巴管瘤的CT表现及诊断价值。材料和方法：经手术和病理证实腹部淋巴管瘤28例，均行CT平扫和增强扫描，分析其CT表现，并与手术、病理对照。结果：单房型淋巴管瘤13例；多房型淋巴瘤15例。16例囊壁及间隔清楚显示，12例囊壁及间隔未显示，全部病例均未见壁性结节。腹腔内淋巴管瘤18例，其中11例发生于小肠系膜；腹膜后淋巴管瘤10例，其中有5例发生于肾上腺区域。结论：肠系膜和肾上腺区域分别是腹腔和腹膜后淋巴管瘤的好发部位。CT能准确显示淋巴管瘤的大小、形态、囊壁和分隔，明确其囊性性质，CT值偏低或穿刺液为乳糜性具有较大定性诊断价值。

CT Diagnosis of Abdominal Lymphangioma

Objective:To study the CT features of abdominal lymphangioma and to evaluate the diagnostic value of CT findings.Methods:28 cases of abdominal lymphangiomas were proved surgically and pathologically.Plain and contrast scan were performed in all cases.The CT signs were analyzed retrospectively and comparisons were made between these signs and surgical and pathological results.Results:13 of 28 cases revealed as monolocular type with round or oval shape.15 of 28 cases revealed as mutilocular type with interstitial involvement.Septum and wall of the lymphangiomas were clearly delineated in 16 of 28 cases,and were not visible in the remaining 12 cases.No mural masses were detected in all cases.18 of 28 lymphangiomas were from peritoneal cavity (11 cases were from mesentery).10 of 28 lymphangiomas came from retroperitoneal space (5 cases come from adrenal area).Lymphangiomas were well demarkated with fluid density.Conclusions:The lymphangiomas from peritoneal cavity were more commonly in the mesentery,and the lymphangiomas from retroperitoneal space were more frequently in the adrenal area.CT was capable of revealing the size,the shape,the cystic wall and the septum of lymphagioma,and was helpful in determining the cystic nature of the lesion.Chylous effusion and low CT value were valuable for the diagnosis of lymphangioma.
