| 摘 要： | 目的观察卡维地洛治疗慢性充血性心力衰竭(CHF)的疗效及其对血浆脑钠素(BNP)和内皮素-1(ET-1)的影响。方法 70例CHF患者随机分为治疗组与对照组,治疗组在常规治疗的基础上,从小剂量开始加用卡维地洛至目标剂量维持;对照组仅采用常规治疗。治疗6个月后观察疗效,测定用药前和用药6个月后血浆BNP与ET-1水平的变化,并与对照组进行比较。结果治疗6个月后,治疗组总有效率为89%,明显高于对照组的63%,有显著性差异(P<0.05)。治疗组治疗后左室舒张末内径(LVEDD)和左室收缩末内径(LVESD)均较治疗前明显缩短(P均<0.01),左室射血分数(LVEF)较治疗前明显升高(P<0.05);与对照组比较,治疗组治疗后LVEDD和LVESD缩短均更为显著,而LVEF升高则更为明显,均有显著性差异(P均<0.05)。治疗后2组血浆BNP与ET-1水平较治疗前均显著下降;与对照组比较,治疗组治疗后血浆BNP与ET-1水平下降均更显著(P均<0.05)。结论卡维地洛治疗CHF能改善心功能,改善左室重塑,使用安全,疗效显著。其机制可能与降低血浆BNP和ET-1水平有关。
