保守治疗异位妊娠的临床疗效观察

目的探讨保守治疗异位妊娠的最佳方案。方法回顾性分析168例保守治疗的异位妊娠,随机分为A、B、C三组,A组：甲氨蝶呤50mg/m2肌注;B组：米非司酮片每天空腹口服50mg,2次/d,共3天,第4天行甲氨蝶呤50mg/m2肌注,共一次;C组：甲氨蝶呤50mg/m2肌注,共1次,并口服中药治疗;定期检测血β-HCG水平及阴道B超监测包块,直至正常。结果 C组治愈率显著高于A、B两组（P〈0.05或0.01）,而A、B两组差异无显著性P〉0.05;A组的不良反应发生率显著低于B、C组（P〈0.05或0.01）,C组显著低于B组P〈0.05;血β-HCG一周下降〉15%,C组明显高于A、B两组（P〈0.01）。结论甲氨蝶呤内注射配合中药治疗异位妊娠疗较好,不良反应小,可作为首选治疗方案。

The Conservative Treatment of Ectopic Pregnancy Clinical Efficacy

Objective To study the efficacy of the conservative treatment in the treatment of ectopic p regnancy.Methods A retrospective analysis of 168 patients of conservative treatment of ectopic pregnancy were randomly divided into A,B,and C.Group A： methotrexate 50mg/m2 intramuscular;group B： mifepristone tablets fasting oral administration of 50 mg per day,2 times/d,a total of three days,the fourth day the line methotrexate 50mg/m2 intramuscular injection;group C： methotrexate 50mg/m2 intramuscular,and oral medicine treatment;periodic testing of serum β-HCG levels and vaginal B monitoring mass until normal.Results The cure rate of group C was significantly higher than A,B groups（P0.05 or 0.01）;incidence of side effects in group A was significantly lower than B,Group C;serum β-HCG week dropped by15% of patients the number of group C was significantly higher than A and B groups（P0.01）.Conclusion Methotrexate injection combined with chinese medicine treatment of ectopic pregnancy treatment is better,less side effects and can be used as the preferred treatment options.
