儿童过敏性紫癜肾脏损害的危险因素分析

目的探讨儿童过敏性紫癜肾脏损害的危险因素。方法将160例过敏性紫癜住院患儿分成有肾损害组(n=69)和无肾损害组(n=91),采用χ2检验及秩和检验,有肾损害组和无肾损害组为应变量,差异有显著性的临床指标和年龄为自变量,进行逐步logistic回归分析肾脏受损的危险因素。结果43.1%患儿发生肾损害,单因素分析提示反复性皮疹、消化道出血及腹痛在有肾损害组和无肾损害组两组之间差异有显著性(P<0.05),经逐步logistic回归分析表明反复性皮疹和腹痛是过敏性紫癜肾脏损害的危险因素(P<0.05)。结论反复性皮疹、腹痛是过敏性紫癜肾损害的危险因素,因此对于有上述表现的过敏性紫癜患儿更应加强随访。

Risk factors of renal damage in children with Henoch-Schonlein purpura

Objective To study the risk factors of renal damage in children with Henoch-Schonlein purpura(HSP). Methods 160 cases with HSP were retrospectively studied. The patients were divided into 2 groups, HSP without renal damage (n=69) and with renal damage(n=91). Univaiate and multivariate analysis (binary logistic regression) were performed to assess the risk factors of renal damage. Results Sixty-nine cases(43.1%) of the 160 cases had renal damage. The univariate analysis revealed that purpura relapse, gastrointestinal bleeding and abdominal pain in HSP with renal damage were different from that in HSP without renal damage(P<0.05).Logistic regression analysis found that purpura relapse and abdominal pain were the risk factors of the renal damage(P<0.05). Conclusion Purpura relapse,and abdominal pain are the risk factors of the renal damage. Patients with these clinical manifestations should be rigorously monitored.
