痔上黏膜环形切除在重度内痔治疗中的应用

目的探讨痔上黏膜环形切除在重度内痔手术治疗中的临床效果。方法选择106例重度内痔患者随机分为对照组及观察组，两组患者均采用手术切除痔核，观察组切除痔核后在肛门镜下行痔上黏膜环形切除术，比较两组患者术中及术后恢复情况，对两组患者规律随访12个月，观察两组患者复发情况。结果观察组手术时间长于对照组（P〈0．05），术中出血量高于对照组（P〈0．05），两组术后第3d手术部位疼痛、出血、坠胀、水肿评分差异无统计学意义（P〉0．05），术后12个月观察组患者复发率低于对照组（P〈0．05）。结论在痔核切除基础上行痔上黏膜环形切除治疗重度内痔能够降低复发率，改善临床效果，手术安全有效。

Application of Loop Excision of Hemorrhoid Mucosa in Treatment of Patients with Severe Internal Hemorrhoid

Objective To investigate the effects of loop excision of hemorrhoid mucosa in treatment of pa-tients with severe internal hemorrhoid. Methods One hundred and six patients with severe internal hemor- rhoid were divided into observation and control groups. All of the 106 patients were operated for hemor- rhoids removals, besides this, the patients in the observation group were performed additionally with loop excision of hemorrhoid mucosa by anoscope. The recovery situations in operation and after operation were compared between the two groups, as well as the relapse rates in 12 months--followed up. Results The operation time was longer in the observation group than that in control group （ P 〈0.05）, and the hemor- rhage volume in operation was much more in the observation group than that in control group （ P 〈0. 05 ）. The relapse rate was lower in the observation group than that in control group （ P 〈0.05）. Howev- er, there was no statistical difference in pain, hemorrhage, tenesmus and edema in operative site at three days after operation between the two groups （ P 〉0.05 ）. Conclusion After removal of hemorrhoids, loop excision of hemorrhoid mucosa can decrease the relapse rates in patients with severe internal hemorrhoid, improve the clinical outcomes effectively and safety.
