周围神经选择性切断治疗脑瘫性下肢痉挛

目的探讨周围神经选择性切断治疗脑瘫性下肢痉挛的疗效。方法回顾2003年1月至2008年3月收治的380例脑瘫患者,根据痉挛部位不同,选择相应周围神经进行部分切断,包括：腰骶段脊神经后根、坐骨神经、闭孔神经和胫神经。结果术后痉挛缓解率100%,其中82例患者肌力下降,142例患者出现肢体麻木、感觉减退,随访期间,均见好转。随访6个月至3年,随访期间痉挛缓解率为95%。95%患者行走步态好转。结论周围神经选择性切断术是治疗脑瘫性下肢痉挛的安全、有效的神经外科方法。

Selective peripheral denervation for treatment of lower limbs spasm of cerebral palsy

Objective To discuss the therapeutic effect of selective peripheral denervation for lower limbs spasm of cerebral palsy. Methods All 380 patients with cerebral palsy, admitted from January 2003 to March 2008, were studied retrospectively. Based on the position of spasticity, selective peripheral denervation was performed, including spinal dorsal root of lnmbosacral region, sciatic nerve, obturator nerve, peroneal nerve. Results Instant post-operational relief rate was 100%, including decreased limb strength in 82 cases and numbness or decreased sensitivity in 142 cases. All cases were followed-up for 6 months to 3 years and got a relief rate of 95%. A total of 95% cases improved in gait. Conclusion Selective peripheral denervation is safe and effective in treating spasticity of lower limbs caused by cerebral palsy.
