诺维本联合顺铂治疗晚期乳腺癌的疗效分析

目的:观察去甲长春花碱(NVB,诺维本)联合顺铂(NP方案)治疗转移性乳腺癌的近期疗效和毒副作用。方法:对38例转移性乳腺癌患者采用诺维本联合顺铂方案(NVB25mg/m2,静脉滴入,d1,d8;DDP30mg/m2,静脉滴入,d1～3,21d为一周期)治疗。38例患者均可疗效评价,化疗共95周期,中位数为2.5周期(2～4周期)。结果:38例转移性乳腺癌患者中总有效率(CR PR)为57.9%(22/38),其中CR4例,PR18例。中位缓解期6.8个月(1～17个月)。该方案的主要不良反应以骨髓抑制为主,白细胞减少的发生率为78.9%;其次为消化道反应;偶有静脉炎的发生。本组患者绝大多数采用深静脉给药方法(锁骨上静脉置管给药),故静脉炎的发生率低。无治疗相关死亡病例,耐受良好。结论:去甲长春花碱联合顺铂方案(NP方案)治疗转移性乳腺癌疗效较好,而且不良反应较轻,应用安全。

Combination chemotherapy with vinorelbine and cisplatin in the treatment of advanced breast cancer

Objective:To observe the recently therapeutic effects of NVB combined with DDP in the treatment of recurrent or metastatic breast cancer.Methods:Thirty eight patients of recurrent or metastatic breast cancer were treated with NVB 25 mg/m2 IVD for day 1 and day 8,and cisplatin 30 mg/m2 IVD for day 1 to day 3,21 days a cycle.Therapeutic effects of 38 cases could be evaluated.Total 95 cycles were given with a median of 2.5 cycles(2-4cycles).Results:Among the 38 cases the overall response rate(OR)was 57.9%(22/38),including CR 4 cases,PR 18 cases.The medium duration of response for all patients was 6.8 monthes(1~17 monthes).The main toxic effects were bone marrow suppression,mainly leukopenia in 78.9%.And next was troubles in gastrointestinal tract.The incidence of phlebitis was very low,since in most cases drugs were given through subclavicle vein intubation.There were no treatment related deaths and the treatment had good tolerability.Conclusion:For recurrent or metastatic breast cancer,NVB combined with DDP is an effective regimen of treatment,and the regimen is safe with a tolerable toxicities.
