消化道癌合并糖尿病的围手术期处理

目的：探讨消化道癌合并糖尿病患者围手术期处理的效果。方法：回顾性分析2004年9月到2009年9月太钢总医院普外科收治的合并糖尿病胃癌及大肠癌患者的临床资料。结果：胃癌及大肠癌合并糖尿病患者151例中142例行择期手术，9例行急诊手术。术后并发症：吻合口瘘5例占3．3％，切口感染22例占14．6％，合并泌尿系感染4例占2．6％，腹腔感染6例占4．0％。术后平均住院23d。围手术期死亡1例。结论：胃癌及大肠癌合并糖尿病的患者只要在围手术期严格控制血糖，选择合理的手术方式，可达到很好的疗效。

Perioperative Treatment of Gastrointestinal Cancer Combined with Diabetes

Objective： To investigate the effect of the perioperative treatment for the patients with gastrointestinal cancer combined with diabetes. Methods： The clinical data of the patients with gastric cancer and colorectal cancer who combined with diabetes admitted and cured in general surgery department of General Hospital of Taiyuan Iron and Steel from September 2004 to September 2009 were analyzed retrospectively. Results： One hundred and forty-two patients were performed by elective surgery and 9 patients were performed by emergency surgery among 151 patients with gastric cancer and colorectal cancer who combined with diabetes. The postoperative complications were that anastomotic leakage accounted for 3.3% in 5 cases,wound infection was 14.6% in 22 cases, combined urinary tract infection was 2.6% in 4 cases, abdominal infection was 4.0% in 6 cases. The mean postoperative hospital stay was 23 days. Perioperative death was one case. Conclusion： The patients with gastric cancer and colorectal cancer who combined with diabetes may achieve good therapeutic effectiveness if their blood glucose could be strictly controlled and reasonable surgical approach could be chosen.
