妇科腹腔镜与剖腹手术对机体凝血系统影响的比较研究

目的研究腹腔镜手术和剖腹手术对机体凝血系统的影响。方法将60例需行妇科手术的患者分成两组，腹腔镜手术组30例（N1组）和剖腹手术组30例（N2组），采用法国STA—Compact全自动血凝仪检测60例患者术前、术后24h及48h静脉血清的凝血酶原时间（PT）、活化部分凝血活酶时间（APTT）和纤维蛋白原（FIB）水平，并对两组结果进行比较分析。结果两组术后24h及48hPT、APTT比术前明显降低，FIB明显升高（P〈0．05），但N1组和N2组组间差别无统计学意义（P〉0．05）。结论腹腔镜手术和剖腹手术均使患者机体处于高凝状态，但尚不能得出腹腔镜手术后患者血栓性疾病的发生率比剖腹手术低的结论。

Comparative analysis of laparoscopic surgery and laparotomy for coagulation system in patients

Objectives To study the effects of laparoscopic surgery and laparotomy for coagulation system in patients. Methods Sixty patients were divided into the laparoscopic surgery (N1 group) and laparotomy (N2 group) groups with 30 in each group. The plasm prothrombin time (PT),activated partial throm boplastic time (APTT) and fibrinogen (FIB) in serum from venous blood at 24 h and 48 h after operation were measured by STA-Compact automated coagulation analyzer. Results The contents of PT,APTT decreased and FIB increased markedly at 24 h and 48 h postoperatively in both N1and N2 groups (P <0.05),but there were no significant difference between N1and N2 groups (P >0.05). Conclusion Both laparoscopic surgery and laparotomy made coagulation stata vary significantly,but there was no evidence showed that laparoscopic surgery caused less effects on Deep Venous Thrombosis than laparotomy in patients.
